Medical Model

The weekly food delivery program is designed for medically at risk clients who are in need of fresh, nutritious foods and are unable to come to the SAN office. The clients are assessed by a Registered Dietitian to determine their eligibility.  Clients are followed on an ongoing basis by the dietitian. There is no income eligibility for this program. Food bags include at least 2-4 sources of protein as well as a variety of foods to make 6-7 balanced meals. The foods are assembled and delivered by volunteers to the clients' homes every Thursday afternoon. For many of these clients, this is the only contact they have throughout the week.
