Case Management

SAN case managers assist clients to acquire and maintain medical insurance, schedule appointments with an HIV specialist, and guidance on treatment adherence. Case managers can also help clients with the following: 
  • Basic needs such as food, clothing, household items 
  • Referrals for emergency funds 
  • Mental Health referrals
  • Substance abuse referrals 
  • Housing referrals
  • Assistance with transportation 
  • Assistance with legal needs such as Power of Attorney or Living Will 
  • HIV education and prevention 
  • Assistance with employment/income such as Back to Work, GAU, Social Security
